| Part 3: The Questionnaire This survey is very simple. Interviewers will ask between 1 to 3 questions as follows: 1.
"Did you drive to the ballpark today?" If the person
replies that they did not drive, then
thank
the person. You have now finished the survey with this
respondent.
Circle the number "2" to the right of "Did not drive" on the
questionnaire. If the answer is
"yes", proceed to Question #2.
2. "Were you the driver or a passenger?" . If they say they were a passenger, then thank the respondent and circle "2" next to "Did not drive" on the questionnaire. You have now completed the interview with the respondent. If they were the driver, circle the number "3" to the right of "Drove - confirmed" on the questionnaire and move on to Question #3. 3. "At about what time did you park?" Circle the number to the right of the appropriate response under the heading "Parked". |

Referring back to the questionnaire, notice that there are six boxes on the page. Each box is designed to contain responses for one respondent.
Please note that if the 25th person through your turnstile is under the age of 16 years old, you do not need to interview that person. You would then circle the number "1" to the right of "Age 3-15, not interviewed", which you can see is at the top left corner inside each box.
